Every
order of Medical ID jewelry is accompanied with our free Medical
ID wallet Card. In many cases it is impossible to engrave all
needed information of Medical ID bracelets or Dog Tags. Some
textual medical information can be long enough to be placed on
ID plates.
If this is a case, our Free
medical ID wallet card will be a handy addition. Pictures below
illustrate which additional medical information can be added to
the card.
There is place for personal medical
information as well as whom to contact in case of Emergency. If
you have more than one doctor who have to be called at the time
of need, you will find a second line for another doctor.
Also, your critical allergies and
medications you take can be specified on the back side of the
Medical ID wallet Card.
Finally your blood type and any
other important information can be found on the Medical ID
